Middle Block - Key Points
Ensure that the radial and ulnar arteries are identified and that they are not in the path of the needle.
The median nerve divides into the palmar and dorsal branches in the distal third of the forearm. Therefore, it is advisable to perform the block in the medial or proximal third of the forearm.
Volume of anesthetic required to complete the block: 6-8 mL.
The goal is to apply the anesthetic so that it covers the nerve 66-75%. Performing a "donut" is not necessary and increases the risk of vascular or nerve punctures. Generally, as soon as you are in the right space, you can apply your local anesthetic.
